Ask about McDVoice without receipt and you are usually holding one, with a 26-digit survey code that has faded past reading. That case has a fix. A link on mcdvoice.com covers receipts printed with no readable code, and the form behind it wants the store number, the KS number, the date, the time, the order number and the amount spent. A binned receipt has no route at all.

How to take the survey
Work out which of the two problems you have
A code you cannot read and a receipt you cannot find are different failures. One has a published route. The other has none, and no amount of searching produces one.
Photograph the paper before you do anything else
Thermal paper carries heat-developed dye rather than ink, and it keeps reacting after printing. A phone photo survives everything a glovebox does to the original.
Take the no-code link under the six boxes
A line sits beneath the code fields for receipts without a readable 26-digit survey code. That branch belongs to the official form, not to folklore.
Read the six details off the same slip
Store number, KS number, date, time, order number and amount all print in ordinary type among the order lines, away from the dense block that fades first.
If the receipt has gone, stop
No customer service line reissues a survey invitation. Your next visit prints a fresh one, along with a fresh 26-digit survey code.
Send the comment anyway if it matters
mcdonalds.com takes comments through a customer feedback form with no receipt involved. Nothing is rewarded that way, and nothing ever has been.
If something goes wrong
Half the digits are ghosts on the paper
- Cause
- Thermal paper carries dye rather than ink, and heat, daylight and friction all keep developing it
- Fix
- Take the no-code link on mcdvoice.com and read the store number, KS number, date and time off the same slip.
I threw the receipt away
- Cause
- The validation code redeems an offer printed on that specific paper
- Fix
- Nothing reissues it. Your next visit prints a fresh invitation, and the monthly count is per restaurant.
The alternate link is not on my entry page
- Cause
- Receipt formats differ, and the branch shows for those that need it
- Fix
- Look under the six code boxes before deciding it is missing.
A page offered me the survey with no receipt
- Cause
- No prize draw sits behind McDVoice, so there is no free entry route for such a page to describe
- Fix
- Close it. The genuine survey never asks for a card number, a Social Security number or a fee.
My store number keeps being rejected
- Cause
- The street address number was typed in place of the store number
- Fix
- The store number sits with the KS number near the visit details, and it is digits only.
A code somebody else gave me failed
- Cause
- Section 4.2 of the Survey Page Terms of Service says a code cannot be used again once it has been entered
- Fix
- One code, one response. Only a fresh receipt produces a fresh entry.
What’s fixed, and what actually varies
Two things here come from McDonald's own survey page and one from the terms document that page links. What is left depends on which franchisee printed your paper.
Fixed
- mcdvoice.com carries a link for receipts printed without a readable 26-digit survey code
- The validation code redeems the offer printed on your receipt, which is why a lost receipt ends it
- Five surveys per restaurant each month, stated on the entry page
- 13 is the minimum age in the Survey Page Terms of Service, section 5.1
- No sweepstakes runs, so no free alternate method of entry exists to find
Varies
- Whether the no-code branch appears for your receipt format
- Whether your franchisee prints a survey invitation at all
- The offer itself — a free item, a buy-one-get-one or a discount
- How long a written validation code stays good, reported between 7 and 30 days
- Whether redeeming it needs a purchase
The line under the six code boxes tells you whether the alternate branch exists for your receipt. The line under the survey invitation on the paper tells you what you stand to get. Between them that is about ten seconds of reading.
How McDonald's compares
Missing paperwork breaks these surveys in different places, and the fallback is where they diverge.
| Chain | If the code is unreadable | If the receipt is gone | Free entry route |
|---|---|---|---|
| McDonald's | Second form takes store number, KS number, date, time, order number | No route | None, because no draw runs |
| Wendy's | No code to lose; three receipt fields instead | No route | None published |
| Taco Bell | Entry form opens on store number, date and time | No route | Mail-in, named in the sweepstakes rules |
Offers vary by restaurant and by period; the paper in your hand overrides this table.
McDonald's publishes the clearest fallback of the three for a code nobody can read. Only Taco Bell has ever carried a documented free entry route, and that belongs to a prize draw rather than to the food offer.
A faded code and a lost receipt are not one problem
Two different failures arrive under one search term, and only one of them has an answer.
The first is a receipt whose survey code has stopped being legible. Thermal paper carries no ink at all. The image is heat-developed dye that keeps reacting to warmth, sunlight and the friction of a pocket, so a slip left on a dashboard in July can go blank in an afternoon. McDonald's designed around that. Checked on 25 August 2026, the entry screen carries a line under the code boxes for anyone whose receipt has no 26-digit survey code on it, and the form behind that link asks for details sitting elsewhere on the same paper: store number, KS number, date, time, order number and amount spent.
The second failure is a receipt that no longer exists, and there the answer is short. Nothing recovers it. According to mcdvoice.com, finishing the survey produces a validation code that redeems the offer printed on your receipt, so the reward is a specific line on a specific slip rather than a general prize.
That wording also disposes of the workaround people look for next. A prize draw in the United States has to publish a free alternate method of entry, which is where the familiar mail-in route comes from. McDVoice publishes no draw whatsoever: no sponsor, no entry period, no prize, no odds, no winners list. With no draw there is nothing of the sort to find, and a page offering you one is describing something McDonald's does not operate.
What remains is worth knowing. The five-a-month cap runs per restaurant rather than per person, so a different McDonald's carries its own count, and mcdonalds.com takes comments through a customer feedback form that has never carried a reward.
Questions people ask
Can I take the McDVoice survey with no receipt at all?
No. The validation code redeems the offer printed on the receipt, so with no receipt there is nothing to unlock. No customer service route starts one for you.
My 26-digit survey code has faded. Is that the same thing?
It is not, and the difference matters. The entry page carries a link for receipts with no readable code, and the form behind it takes the store number, the KS number, the date, the time and the order number.
Is there a mail-in or free entry route?
None exists. Free entry routes belong to prize draws, which US law requires to publish one. McDVoice names no sponsor, no entry period, no prize and no odds, because no draw sits behind it.
Can I use a receipt somebody else was given?
Only if nobody has used it. Section 4.2 of the Survey Page Terms of Service says an entered code will not work a second time.
How long does a receipt stay usable?
Receipts print seven days. The entry screen at mcdvoice.com states no window at all, so guides quoting the figure are quoting the paper rather than the site.
My receipt has no survey invitation on it. Now what?
That franchisee is not running the offer, and no branch of the entry form covers a receipt with no invitation. A different McDonald's may well print one.
Can I still tell McDonald's what went wrong?
Yes. mcdonalds.com carries a customer feedback form needing no receipt and no code. It attaches no reward, and Service Management Group has nothing to do with it.
